Third Sector Hospital Liaison Worker (Western General)

Hours:  18 hours per week, to be worked across five mornings
Salary: £26,335 pro rata (fixed point)
Contract: until 31st March 2018 with possible extension dependant on funding

Health in Mind delivers the LOOPs Hospital Discharge Support Project within the Western General Hospital.  The Third Sector Hospital Liaison Worker will assess people referred to the service and provide support to older people, linking them in with appropriate community support and services.

If you have experience of supporting older people. service promotion, working with multidisciplinary teams, delivering information about and making referrals to services, we'd love to hear from you.  This post will appeal to someone who can demonstrate resilience and a pioneering attitude, key to developing a new service.
